Clause 6. Work permit grantees wishing to apply for (making) amendment(s) to or adding of job type or description shall proceed as follows:

  1. Foreigner work permit grantees under Clause 1 (1) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b), (c), (d) and (e)
  2. Foreigner work permit grantees under Clause 1(2) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b), (c), (d), and Clause 1 (2)(b)
  3. Foreigner work permit grantees under Clause 1 (3) shall apply using Annex Form 6 annexed to this Ministerial Regulation, including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b), (c), (d) and Clauses 1 (3)(b), (c) and (d)
  4. Foreigner work permit grantees under Clause 2 shall apply using Annex Form 2 annexed to this ministerial regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(b), (d) and Clause 2 (1)

Clause 7. Work Permit grantees wishing to apply for making amendments to or adding of employers shall proceed as follows:

  1. Foreigner work permit grantees under Clauses 1 (1), (2) or (3) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1), (2) or (3), depending on the case/as appropriate
  2. Foreigner work permit grantees under Clause 2 shall apply using Annex Form 2 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clause 2

Clause 8. Work permit grantees wishing to apply for making amendments to or adding (of) region or location of employment/premises of business shall proceed as follows:

  1. Foreigner work permit grantees under Clause 1 (1) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b) and (e)
  2. Foreigner work permit grantees under Clause 1 (2) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b) and Clause 1 (2)(b)
  3. Foreigner work permit grantees under Clause 1 (3) shall apply using Annex Form 6 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a), (b) and Clauses 1 (3)(b), (c) and (d)
  4. Foreigner work permit grantees under Clause 2 shall apply using Annex Form 2 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(b), (e) and Clauses 2 (1) and (3)

Clause 9. Work permit grantees wishing to apply for making amendments to or adding of the conditions/particulars of the permit shall proceed as follows:

  1. Foreigner work permit grantees under Clauses 1 (1), (2) or (3) shall apply using Annex Form 6 annexed to this ministerial regulation including certificates or licenses, documents and evidence specified in Clauses 1 (1)(a) and (b)
  2. Foreigner work permit grantees under Clause 2 shall apply using Annex Form 2 annexed to this Ministerial Regulation including certificates or licenses, documents and evidence specified in Clause 1 (1)(b) and Clause 2 (1)

Article 2

Application for Work Permits, Application for Renewal of Work Permits, Application for Substitute Certificates for Work Permits under Section 13, Application for Making Amendments to or Adding of Job Type or Description, Employers, Region or Location of Employment, or Conditions/Particulars of the Permit

Clause 10. Foreigners that were deported per the Deportation Act and that were permitted to work in a location other than the country of deportation or those who are in the process of deportation and wish to receive a work permit under Section 13 shall apply using Annex Form 7 annexed to this Ministerial Regulation, including documents and evidence as follows:

  1. Copy of document from a government administrative body/government administration certifying that the applicant is a foreigner who has been deported per the Deportation Act and has been permitted to work in a location other than the country of deportation or is in the process of deportation
  2. Copy of evidence of domicile/residence per regulations issued by the Director-General
  3. Copy of educational qualifications or reference documents from a previous employer specifying details concerning job description and period of employment, or a reference letter from the applicant’s future employer indicating applicant’s knowledge and experience being suited to the work seeking permit grant
  4. Copy of License for Professional Practice if the profession seeking work permit grant is required by law to obtain a License for Professional Practice*
  5. Employment contract from future employer indicating grounds for not hiring a Thai national as well as evidence to support such reasons following guidelines issued by the Director-General
  6. Copy of future employer’s identification card and house registration or passport or copy of residence permit in accordance with immigration laws in the case where the future employer is a natural person, or copy of reference letter from appropriate governmental administrations indicating that the (future) employer’s business is correctly registered or licensed to*establish and operate by law. If the employer is a juristic person, the type of business shall also be indicated.
  7. Certificate from a medical professional complying with medical practice laws confirming that the applicant is not an individual who is mentally insane *(incompetent) or who has mental disorders or sicknesses as outlined in the Ministerial Regulation under Section 10
  8. 3 Half-length photos; full-faced, no hat, no dark colored glasses, of 3 x 5cm size and  taken within the past 6 months
  9. Map indicating location of employment

Clause 11. Foreigners that have entered or are resident in the Kingdom of Thailand without permission but have been granted permission to reside in the Kingdom temporarily while awaiting deportation per Immigration laws and wish to apply for a work permit under Section 13 shall proceed as follows:

  1. Foreigners who are Burmese, Laos and Cambodian nationals who have been granted special permission from the Minister of Interior to reside in the Kingdom shall apply using Annex Form 8 annexed to this Ministerial Regulation, including documents and evidence as follows:
    1. Copy of non-Thai national identification or copy of history/*registration document or copy of document certifying *personal records from *a provincial office or regional office issued per Civil registration laws
    2. Certificate from a medical professional complying with medical practice laws from a medical hospital/institution confirming that the applicant does not have the prohibited sicknesses as specified by the Minister of Public Health
    3. Documents and evidence as outlined in Clauses 10 (5), (6), (8) and (9)
  2. Foreigners not classified under Clause (1) shall apply using Annex Form 7 annexed to this Ministerial Regulation, including the following documents and evidence:
    1. Copy of non-Thai national identification, copy of identification for individuals without *registered status or copy of evidence of permit for temporary residence in the Kingdom of Thailand pending deportation out of the Kingdom
    2. Copy of Domicile/House registration
    3. In the case of application for work permit for employment outside of residence or region/district of permit issue; copy of permit for temporary leave of residence or region/district of issue from* the Chief District Officer or the Provincial Governor
    4. Documents and evidence as outlined in Clauses 10 (3), (4), (5), (6), (7), (8) and (9)
